Cash Contributions

Cash contributions (checks or money orders) may be given to any CGMA Representative or mailed to:

• Please make your check or money order payable to Coast Guard Mutual Assistance or CGMA.

• Do not send cash through the mail.

• To help us better understand who is contributing to Coast Guard Mutual Assistance, please complete and return a Cash Contribution Form with your contribution (Appreciated - but not required).

• Contribution Forms are available in Adobe Acrobat (PDF) format.  If you do not have Adobe Acrobat Reader on your computer, you may download it free by clicking here  Get Adobe Acrobat Reader.   Applications may also be obtained from your local CGMA Representative.

  If you wish, you may complete the form online.  Once completed, the form may be printed and forwarded with your contribution.  You should also save a copy of the printed form for yourself.

  If you do not wish to complete the form online, you may simply print either version of the form, complete it manually and forward it with your contribution.
