Memorial Contributions
Contributions may be made "in memory" of another individual. Contributions may be given to any CGMA Representative or mailed to:

Please make your check or money order payable to Coast Guard Mutual Assistance or CGMA.

Do not send cash through the mail.

If you complete and attach a Memorial Contribution Form, Coast Guard Mutual Assistance Headquarters will provide written acknowledgment to you and, if you desire, the individual's next-of-kin.

Contribution Forms are available in Acrobat (PDF) format. If you do not have Adobe Acrobat Reader on your computer, you may download it free by clicking here Get Adobe Acrobat Reader. Applications may also be obtained from your local CGMA Representative.
If you wish, you may complete the form online. Once completed, the form may be printed and forwarded with your contribution. You should also save a printed copy of the form for yourself.
If you do not wish to complete the form online, you may simply print either version of the form, complete it manually and forward it with your contribution.
Contributions to Coast Guard Mutual Assistance, a tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code, are deductible for income and estate tax purposes.
